Enroll in Together with BLENREP
We understand the importance of making the decision with your doctor to start BLENREP. Together with BLENREP is here to help you navigate coverage and provide ongoing support.
Get eye drops at no cost
If you’ve been prescribed BLENREP, you are eligible to receive preservative-free eye drops through the BLENREP Eye Drop Program at no cost. The eye drops will be shipped directly to your home every 60 days for as long as you are on treatment. To enroll in the Eye Drop Program and to receive your eye drops, follow these steps:
- Check the Eye Drop Program box on the Together with BLENREP enrollment form, which you can complete either online or by downloading a form to fill out with your doctor.
- You only need to provide your name, address, phone number, and signature. No other form fields, including insurance information, are required.
- A Nurse Navigator will reach out to you to confirm your prescription and address. Be sure to answer calls from: 945-231-0989.

Get cost support options for BLENREP
Paying for medicine is not one-size-fits-all. The necessary support can be different depending on your eligibility and insurance coverage. Together with BLENREP helps you understand and clarify the various support options that are available to you.
Get a dedicated Nurse Navigator
Your Nurse Navigator can answer questions about insurance benefits and out-of-pocket costs. They can help you understand financial assistance, logistical needs as you start BLENREP, and provide you with BLENREP resources throughout the treatment process.
Get eye care support for BLENREP
Together with BLENREP can help you with finding an eye care professional, if you don't already have one, and with sending reminders about exams. We can also help you continue to manage any insurance needs throughout treatment.
