Together with ZEJULA offers a range of assistance options at no cost to you. After your doctor has prescribed ZEJULA, you can choose how you want to enroll in the program. Follow the steps below to download and complete the enrollment form.
Start enrollment in three easy steps
Download and print
Click “DOWNLOAD” below to download the enrollment form, then print it out.
Complete and sign
Fill out all the required fields, review your answers carefully, then sign it.
Bring it to your doctor
Show the form to your healthcare provider. They will complete the remaining sections and submit it to us on your behalf.

If you are enrolling in Together with ZEJULA with your doctor, you can download and print an enrollment form, which will guide you and your doctor through the steps. Once the form is received and your enrollment is processed, you’ll gain access to the appropriate services.
